Training in San Antonio

San Antonio providers mostly practice in Bexar County and its neighbors, Comal, Guadalupe and Kendall. New Braunfels is about 30 miles up I-35, Boerne is about 30 miles northwest on I-10, and Seguin sits east on I-10. Hill Country practices in Kerrville and Fredericksburg, each roughly an hour out, fall under the same request.

Training happens one of two ways. The instructor travels to you and works with you and your staff in your own practice, on your equipment, with travel in the quote. Or you travel to an instructor’s own working practice. Whichever you choose, it is private, the patients are real and consenting, and the scope is settled before you book.

Who can train? Anyone legally able to provide the treatment in Texas, so it turns on the license you hold. The Texas section lower on this page is the short version of where each license stands, with a link through to the full reading, license by license, sources included.

The area a San Antonio session covers

Bexar, Comal, Guadalupe and Kendall counties, plus Medina, Wilson or Kerr if you practice there. That means Alamo Heights, Stone Oak, Helotes, Leon Valley, Live Oak, Converse, Universal City, Schertz, Cibolo, New Braunfels, Seguin, Boerne, Kerrville and Fredericksburg. Health systems in Bexar County

Around a San Antonio practice you will find University Health, officially the Bexar County Hospital District, whose medical staff are UT Health San Antonio physicians and whose University Hospital is, in its words, “the region’s only Level I trauma center for both adults and children”; UT Health San Antonio, a health science center with six professional schools, medicine and nursing among them; and Methodist Healthcare, a 50-50 co-ownership between Methodist Healthcare Ministries of South Texas and HCA Healthcare, serving South Texas since 1963.

Military medicine at Fort Sam Houston

Brooke Army Medical Center sits at Joint Base San Antonio-Fort Sam Houston and calls itself the largest and most robust military healthcare organization in the Department of Defense. Its Level I trauma center serves San Antonio and the surrounding region and takes both military and civilian trauma patients, and the hospital also serves service members, their families and veterans.

Worth knowing

Brooke Army Medical Center traces its history to 1879, when the first Post Hospital opened on the post as a single-story wooden building. The post’s Station Hospital was renamed Brooke General Hospital in 1942, and became Brooke Army Medical Center in 1946. Source

The market

Bexar County and the three counties beside it, as the Census Bureau, the Texas Medical Board and local institutions count them. Each figure is linked to the page that published it.

One county holds most of the market

Bexar County had 2,160,088 residents on July 1, 2025, by the Census Bureau’s estimate, on about 1,240 square miles of land. Comal had 209,166, Guadalupe 201,111 and Kendall 53,289. Of the 2,623,654 people in the four counties, about 82 percent live in Bexar. New Braunfels, Seguin and Boerne are the edges of this market, not a second center.

Younger in Bexar, older in the Hill Country

Bexar County is 23.7 percent under 18 and 14.3 percent 65 or older. Source Set that against 21.1 and 18.9 percent for the country. Source Cross the county line to the north and the older share climbs past the national one: 20.8 percent of Comal County and 23.9 percent of Kendall County are 65 or older. These figures describe who lives here. They are not counts of patients.

Another language at home, in a mostly U.S.-born county

Of Bexar County residents aged five and older, 37.0 percent speak a language other than English at home, against 22.3 percent nationally. Yet 13.8 percent of the county was born outside the United States, just under the national 14.1 percent. All four figures cover 2020 to 2024. Source Source The gap is the point: many who speak another language at home were born in this country.

146,110 veterans in one county

That is the Census Bureau’s count of veterans living in Bexar County for 2020 to 2024. Source It counts another 19,188 in Guadalupe and 16,430 in Comal. The VA’s South Texas Veterans Health Care System gives its primary affiliation as the University of Texas Health Science Center at San Antonio and has 17 locations, among them the Audie L. Murphy Memorial Veterans Hospital in San Antonio and the Kerrville VA Medical Center.

Where the physicians are

In its May 2026 table, the Texas Medical Board puts 7,175 physicians in Bexar County, about 9 percent of the 78,290 in Texas. It puts 320 in Comal, 141 in Guadalupe and 110 in Kendall, so Bexar has roughly 93 percent of the 7,746 in the four counties. Source For physician assistants, the board’s table for that month shows 1,039 in Bexar County, of 11,765 statewide. Source

A county medical society from 1853

The Bexar County Medical Society says it was established in 1853 as the first county medical society in Texas. It reports a current membership of 6,000 plus physicians, covers Bexar County and seven surrounding counties, and describes itself as one of the largest county medical societies in the United States. Source

The South Texas Medical Center

The San Antonio Medical Foundation, formed in 1947, says it has acquired over 600 acres since the 1950s in what is now the South Texas Medical Center and has given over 180 of them to the University of Texas Health Science Center at San Antonio. By the foundation’s count, the center has over 40 medically related treatment, education and research facilities. Source A study of 2024 prepared for the foundation by The University of Texas at San Antonio counted 30,140 jobs in the center’s core area, not all of them in health care. Source

Offices by type, and where they sit

Bexar County has 1,653 offices of physicians in the Census Bureau’s County Business Patterns county file for 2023, along with 132 offices of chiropractors and 219 offices of physical, occupational and speech therapists and audiologists, which the Census reports as a single group. Source The file counts 148 physician offices in Comal, 65 in Guadalupe and 62 in Kendall. Guadalupe has nearly four times as many residents as Kendall and almost the same number of physician offices. These figures are business locations with paid employees, not a count of clinicians.

Local rules

What San Antonio and Bexar County ask of a practice.

This is not about what your license allows. The state decides that, and the Texas section of this page covers it. Here is the business side for a San Antonio practice as of October 2026: the city, county and state offices that handle the space, the name and the waste. It is a starting point and not legal advice. Confirm each item with the office named.

A Certificate of Occupancy for every business

San Antonio’s Development Services Department says a Certificate of Occupancy is required for all businesses in the City of San Antonio, and that no building or structure can be used or occupied until the department has issued one. In a building with several tenants, each tenant space must obtain its own certificate based on the approved use. Source That is the city’s rule. Outside San Antonio’s limits, ask the city you are in, or the county.

Taking over an existing suite

The department’s first advice to anyone opening or expanding is to find out what types of businesses are allowed at the property. It also says that when ownership of a building or tenant space changes and the occupancy classification stays the same, a new Certificate of Occupancy is not required, and you can request a name change on the existing one. Source How we read it: check the zoning and the current certificate before you sign a lease.

The practice name goes to the county or the state

The City says it does not process DBA registrations and that DBAs are registered with the Bexar County Clerk’s Office. Source The Bexar County Clerk takes certificates for unincorporated assumed business names, and adds that an assumed name certificate is not a business license. Corporations, limited liability companies, professional ones included, and professional associations file theirs with the Texas Secretary of State, which says a law effective September 1, 2019 eliminated the county-level filing requirement for them. Source In New Braunfels, assumed names are among the records of the Comal County Clerk.

One health district for the city and the unincorporated county

The San Antonio Metropolitan Health District describes itself as the public health agency charged by state law, city code and county resolution with providing public health programs in San Antonio and unincorporated areas of Bexar County. It is operated as a City department. Source Comal County has a Public Health Department of its own, in New Braunfels.

Medical waste: who hauls it

The Texas Commission on Environmental Quality says a transporter you hire for medical waste must obtain a registration and keep a record of each waste shipment. A practice that generates 50 pounds or less a month of untreated medical waste may take its own to an authorized facility with no permit or registration, and keeps the same kind of record. Source

Physicians, NPs and PAs

Physicians are licensed by the Texas Medical Board and may delegate a medical act to “a qualified and properly trained person acting under the physician’s supervision” when a reasonable and prudent physician would. Source Texas has restricted practice for nurse practitioners, according to the AANP. Source NPs and PAs provide the medical side of care under a physician’s delegation, and prescribing runs through a prescriptive authority agreement. Source

RNs, chiropractors and physical therapists

How we read the Nursing Practice Act: an RN’s hands-on part starts from a physician’s order or delegation. Source The chiropractic statute allows “nonsurgical, nonincisive procedures” and excludes incisive or surgical ones. Source How we read the Physical Therapy Practice Act: it does not name injections. Source Chiropractors and physical therapists should take any injection course to their board before they book.

Stem cell laws

Three Texas laws, HB 177 (2015), HB 810 (2017) and HB 3148 (2019), cover adult stem cell use, investigational stem cell treatment under an institutional review board, and a physician registry. Source None of their sections mentions platelet-rich plasma. Whether a product or procedure falls under them is a question for your counsel and your board. The FDA’s rules apply in every state on top of Texas law.

What San Antonio providers ask before they book.

How much does training in San Antonio cost?

Training inside your own practice in San Antonio starts at $10,000, with the instructor's travel and lodging included. Training at an instructor's location starts at $5,000. Both are starting points. The quote depends on the discipline, the instructor, how many people are training and how much of the curriculum is built around you.

New Braunfels is between San Antonio and Austin. Which session is mine?

San Antonio. New Braunfels and the rest of Comal County fall under a San Antonio session, and Comal is growing quickly: up 29.6 percent from April 2020 to July 2025, by the Census Bureau's estimate. Source San Marcos, the next city up I-35, is in Hays County and belongs to our Austin page. The Texas rules are identical on both sides of that line.

How far is San Antonio International from downtown?

Nine miles. The San Antonio Airport System's terminal planning manual places San Antonio International (SAT) nine miles north of downtown, with US 281 on its west side and I-410 to the south. Source The City of San Antonio owns and operates it. Source The instructor lands there, and their travel and hotel are part of the quote.

Does San Antonio have rules of its own on scope or stem cells?

No. Neither the city nor Bexar County decides what your license allows. That is set by the State of Texas and its licensing boards, and it reads the same here as in Houston, Austin or Dallas. Do you travel into the Hill Country, to Boerne or Kerrville?

Yes. Kendall County, where Boerne is, is one of the four counties a San Antonio session covers, and Kerr County is included if that is where you practice. Neither is a large medical community: the Texas Medical Board's May 2026 table has 110 physicians in Kendall and 164 in Kerr, against 7,175 in Bexar. I'm not licensed in Texas. Can I train in San Antonio?
